NRx Pharmaceuticals Nears FDA Decision on Preservative-Free Ketamine After Review Clears Hurdles

NRx Pharmaceuticals (NASDAQ:NRXP) said it has completed the first-cycle U.S. Food and Drug Administration review of its abbreviated new drug application, or ANDA, for preservative-free ketamine, with one remaining issue related to the product’s vial connector.

Jonathan Javitt, the company’s founder, chairman and chief executive officer, said the FDA identified no major deficiencies concerning the drug, its ingredients, manufacturing, chemical manufacturing controls or labeling. The remaining matter involves a reviewer’s concern that the vial’s Luer lock tip could deform during use or fail to properly connect with a syringe.

NRx’s ketamine vial uses a twist-off plastic cap and a Luer lock connection intended to enable needleless withdrawal of the medicine. Javitt said the company submitted documentation showing that the vial design has been used in three FDA-approved products that collectively shipped approximately 11.9 million doses over the past 12 months, without complaints, returns, adverse events or recalls.

The company also submitted testing data from seven manufacturing lots, involving 500 vials per lot, or 3,500 total units, with no reported malfunctions. According to Javitt, the FDA asked NRx to submit an attestation from its manufacturer confirming that the vial is produced on the same manufacturing lines, with the same materials and processes, as the three already approved products.

“This attestation is being submitted today,” Javitt said on the call. He characterized the FDA request as a final verification step rather than a requirement for new technical data, while noting that the company had not yet received a new GDUFA action date.

Supply Plans Expand as Company Prepares for Launch

NRx said it is expanding its planned launch inventory in response to market research and expectations for demand. Javitt said the company planned to increase manufacturing orders beyond the first 1 million units previously announced to 5 million units of launch stock. Chief Commercial Officer Glenn Tyson described the target as 4 million to 5 million vials.

The company expects it could begin shipping product within a few days of FDA approval, according to Javitt.

Javitt said ketamine remains on the FDA drug shortage list and is considered a strategic drug product by the military and Department of Veterans Affairs. He said existing shortages are driven primarily by manufacturing capacity rather than a shortage of ketamine active pharmaceutical ingredient.

NRx’s production process uses blow-fill-seal manufacturing rather than traditional glass vial filling, Javitt said. He said the process can manufacture about 1 million units in the time a conventional glass bottling line may produce roughly 10,000 units, while avoiding dependence on pharmaceutical glass supplies.

The company has “very significant amounts” of ketamine active pharmaceutical ingredient in inventory, Javitt said. He added that the company expects its plastic vial format to support better unit margins than glass packaging, though NRx did not provide specific margin guidance or expected pricing.

Commercial Strategy Includes Generic and Branded Products

Tyson said the ketamine ANDA represents the first stage of a broader commercialization strategy that also includes planned branded therapies NRX-100 and NRX-101. He said the company intends to pursue premium, reimbursed branded therapies for severe depression and suicidality if those products receive approval.

Tyson said the combined ketamine and esketamine interventional psychiatry market is growing and could exceed $3 billion in 2027. He said NRx is preparing a commercial model for NRX-100 aimed at broad market access and penetration in interventional psychiatry.

For the proposed generic ketamine product, NRx plans to target hospitals, emergency medicine, pain management, interventional psychiatry and other settings where ketamine is used as a non-opioid anesthetic or treatment option. The initial product would be a 50-milligram formulation, at 10 milligrams per milliliter in a 5-milliliter vial.

Tyson said the commercial team is arranging distributor contracts, implementing systems for government and financial reporting, and establishing pharmacovigilance and medical-information functions. The company is also developing an ordering system designed to verify customer licenses.

NRx expects the preservative-free nature of its ketamine to be noted on carton packaging, Tyson said, though the company does not yet know how prominently it will appear in the FDA-approved label. Javitt said the company does not expect comparative toxicity data to appear in the ANDA label because generic drug labels typically follow the reference listed drug.

Tyson said NRx’s market research has found that potential purchasers place importance on consistent supply and the product’s preservative-free profile. He estimated that roughly 2,500 customers account for the majority of ketamine purchasing in the interventional psychiatry market, which he said includes about 8,000 interventional psychiatrists in the United States.

NDA Work Expected to Resume

Javitt also said NRx had been prepared to submit a new drug application, or NDA, for a separate product in June but focused on resolving the vial issue because the same packaging applies to both products. He said the company now plans to return attention to the NDA and continues to expect it to be addressed within roughly 12 months from June.

The company has discussed with the FDA how real-world evidence from 65,000 patients may be evaluated, Javitt said. He said NRx has presented evidence that it believes demonstrates ketamine’s effects on depression and suicidality, though regulatory outcomes and timing remain subject to FDA review.
